PVG Disclosure

There is a disclosure night for all coaches/officials who do not have a current SYFA disclosure at Broxburn Bowling Club.

 

 

Meeting starts at 7:00pm on Monday 30 January.  Officials must bring the following documents/information along with them on the night to enable the disclosure process to be completed:

 

• Passport
• Driving Licence, picture style licence
• Birth Certificate
• Current utility bill with present address
• Address information for previous 5 years
• National Insurance number
• Your team's SYFA registration number - your team secretary can advise this
