SYFA self disclosure form

Form must be completed and brought along when being disclosed by the Club, WLYFA or WLSDA.  Fully complete the form with all information asked.  Do not omit any detail as this will delay the process or result in your disclosure being rejected.

BACFC volunteer form.

For all new volunteers the attached form from BACFC needs to be completed.  Pass the fully completed form directly to James Pegg, or alternatively to a club official (in a sealed envelope).
