Disclosure evening at Broxburn Bowling Club at 7pm Wednesday 3rd March.

Applicants must bring

birth certificate

driving licence or passport

utility bill   e.g. gas bill

Address information including each postcode where you have stayed for last 5 years

National Insurance number - see your payslip

Applicants who do not have a passport and a photo drivers licence MUST bring two passport sized photos signed on rear by doctor, policeman, councillor etc.

Applicants must complete and bring with them the SYFA self declaration form -  SYFA Self disclosure (89.49 kB)

The self declaration form must be fully completed where asked.  Do not omit any information as this will delay the process or even result in your disclosure being rejected.

Make sure you have your hair done for your ID photograph !
